You have two choices to proceed: 1) Download the application as either a .pdf or Word document format (below), or 2) Order our new Application Kit. The Application Kit is a handy tool for gathering and submitting your application materials. It includes the application, a tabbed three ring binder for easy assembly, and lots of helpful hints. Also, the Application Kit is specific to your type of business entity, which eliminates having to read through instructions or requirements that do not pertain to your type of business. (The required documentation is different for a sole proprietorship than for a corporation, for instance.)

Instructions for downloading: Click the above link. A Microsoft Word document (.doc) will open in your web browser window. SAVE it to your hard drive. Then, open it from your hard drive and type in it as you normally would using your word processing program. Save the file, and print it out to send along with your other application materials.
